Informed Consent

Medical History

Appointments & Cancellations

Appointment Confirmation

All appointments must be confirmed in advance. We will send appointment reminders via phone, email, or text message.

Cancellation Policy

We require at least 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may result in a cancellation fee.

Late Arrivals

Please arrive on time for your appointment. If you arrive more than 15 minutes late, we may need to reschedule your appointment.

Risks & Complications: All aesthetic procedures carry some degree of risk. You will be informed of potential risks during your consultation and in your consent forms. You acknowledge and accept these risks by proceeding with treatment.

Limitation of Liability: To the fullest extent permitted by law, Aesthetic Edge Advanced Injectables and its staff shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services or website.

Aftercare Compliance: You agree to follow all aftercare instructions provided. Failure to comply with aftercare may affect your results and we cannot be held responsible for complications arising from non-compliance.

Photography & Marketing Use

Before/after photographs may be taken for medical records and, with your written consent, for marketing purposes. You have the right to:
